How the referral process works

A clear, step-by-step pathway from first referral through to move-on planning.

1Referral

The referring organisation completes a referral form with the client's circumstances and needs.

2Assessment

We review the referral and arrange an assessment with the client where appropriate.

3Support & risk review

We check the accommodation can safely meet the client's needs and risks.

4Placement & induction

A full induction covering the home, house rules, charges and support arrangements.

5Ongoing support

A tailored package with regular reviews — support can increase or decrease as needs change.

6Move-on planning

Planning begins early to support a move towards stable, long-term accommodation.
